How many photos for PIO Card Application

I'm trying to fill up the application for my daughter's PIO card.
Per the instructions - 3) Attach 4 identical passport size photographs

Should I paste one photo per form in the 1st page (since its to be filled in duplicate ) & send 2 extra?
OR
Should I attach one & send 4 extra photos per form?

Bottom line - how many total photos do I need?

Do I need to make duplicates of the photo copies as well? EG; passports, birth certificates etc.

Also my little daughter doesn't have any "visibly distinguishable mark" -no moles... is it ok to leave it blank?

Paste 1 on each copy on the application form.. i.e 2 photos
just attach one more photo by a U pin on each application(do not staple)
so total 4 photos

Make 2 sets of the documents you are sending and club them together with each application copy.

It is ok to leave it blank but preferable to say "none"
