The form instruction clearly states "You must submit original I94" any advice?
Instructions are outdated.
You do NOT need to send original I-94. Just send copy.
how about change of status from b-2 to f-1. Is submiting I-94 copy only ok?
Is a change of status from f-2 to H-1 possible?

Thanks, it also says E-ticket wont work. But i cant get a paper ticket from the airlines. I have only E-ticket, what can i do?
You need to make sure that you get paper tickets while buying from the travel agent. They don't allow eTickets.
Copy of I-94 is OK for any status change or extension.
