Hello
I have obtained affidavit from my mother for my birth as my birth was not regitered. The
Birth certificate affidavit does not mention Mother's maiden name in it. Is it a must? Also
Does the affidavit should contain the names of both parents? I used the sample affidavit
from this affidavit under I-485. Is that Ok? Please help. BTW I will do cp in Montreal.
