Hello,

My parents had visited to US 3 years ago and at that we had applied for their visa extension. The visa extension was denied. they were on tourist visa.
They have 10 years multiple entry visa on their passport.

Now they want to come back to US and visit us again. Is there any restrictions on their re-entry since their extension was denied few years ago? What should we be aware of or concerned about prior to their re-entry ?

If they received the denial before the date on the I-94 and they left US before the date on their I-94, there is nothing to worry about.
But if they received the denial after the expiry date on their I-94,
they may have to prove to the officer that they left soon after receiving
the denial notice. If asked, they may have to show the denial notice, a copy of their
last I-94 and proof of departure(Indian arrival stamp on passport).

Do they cancel 10yrs multiple entry visa if u stay here waiting for the extension though ur I94 is expired and you do not get the extension?

They had received the denial after there I-94 had expired.
They left within few days for India after they received the denial notice.

They do have all the proof of travel documents that they left soon after the letter of denial.
Will they need to do anything in India prior to coming back to US ? do they need to go again and get the visa stamped in India ?

please advise.

No, they do not have to do anything.
Just let them carry all the documents I mentioned to the US . They may not even be asked any questions. If they are, they can convince the officer with the evidence.

If your I94 is expired, you are supposed to leave soon after you get the denial notice.
If you stay on too long , 222g will apply and your visa will get automatically cancelled
and you will have problems entering on the same visa again.
If you are able to show to the officer at re-entry on the same visa, proof that you left within a reasonable time after you got the denial, he will let you through and will not cancel the visa.
