Hello
In my wife's birth certificate her name is not mentioned. I went through the postings and I understand I need to get an affidavit. Do I also have to get a certificate from the authorities why Name is not present in the birth certificate. Has anyone obtained a certificate like this. Thanks for the help.

You don't need to get a certificate regarding why that name is not there.
Mention all that in the affidavit.

IF your Wife name is NOT mentioned in her Birth Crtificate, then How you will PROVE that this BC belongs to her without her name on it? YOu should Get affidevt for her BC from her parents. but not for that why her name is not on BC.

I am in a similar situation. I have my original birth certificate but my name is not on it. Based on my attorney's suggestion, I got two affidavits (one each from my father and mother), stating very clearly when I was born and also the fact that due to hindhu customs, the baby is named only after 10 days and hence the birth certificate does not have my name on it.
The affidavit itself was done before a magistrate.
If you need any info, I can post the draft of the affidavit.

Do NOT mention that particular attorney's name(who happens to be your attorney), else you won't be allowed to post here again.
