Hi,
I am on H1 and wife on H4 and have a US Born Baby. Please could ImmiHelp clarify my questions for PIO application which I have to send to SFO consulate.

Documents to send:
1. Completed PIO Card Application (Original) with photo
2. Completed PIO Card Application (Duplicate) with photo
3. Two additional Photos
4. Money Order for $175 ($155 PIO Fee + $20 Mail back charges)
5. Notarized Photo Copy of my daughter's US Passport (1st Page)
6. Notarized Photo Copy of my daughter's Birth Certificate
7. Notarized Photo Copy of my wife's Indian Passport (1st and last page)
8. Notarized Photo Copy of my Indian Passport (1st and last page)
9. U.S. Address Proof (Copy of Utility Bills)
10. Indian Address proof - My address on passport and wife's address on passport if not our permanent address. What do I do? Do I need to provide this? If yes, what proof should I have to show?

Also I have some doubts in the form as mentioned below:
a> Q-1 - Where do we mention middle name of child?
b> Permanent address in India verifiable. My permanent address in India is not what I have in passport. What should I do?
c> By birth my daughter is US citizen. Q-9b asks - Date on which nationality was acquired. Should this be same as date of birth?
d> Q-14 - Whether the applicant or either of his/her parents or any of his/her grand parents or great grand parents was born and was permanently resident in India as defined in the Government of India Act, 1935 * at any time ? - What question is this? It is just so confusing.
e> Name & complete address of two persons known to the applicant living in India. Shouldn't the details of myself and my wife be sufficient?
f> For small baby should be put thumbprint in place of signature? If yes, which thumb? Right or left?

Please could you clarify my above questions.

I am also in process to get PIO card for my son.

This might help you to get most of your answers. can you please tell me what size of passport photo they need?

>>>>I think you could provide your parent's or your spouse's parents address. I think they need an address that they can verify and some one who can say they know the applicant at the address. You should be able to provide some utility bills from them as proof, I think.

to answer missing questions.

Mention the address that is applicable now dont worry about the different one in passport.

You dont need to mention the date acquired because that question is not applicable. part A asks if the nationality at birth was different then mention it..
\Not applicable here is it.
