I have a SSN card which was issued when I was on a work visa and it says with work authorization only or something like that, now I have a GC so which form do I need to fill to get a new card which has no such restrictions

you dont need to but it helps when dealing with finicky employers.

go to ssn office with GC they will help you. also check ssa.gov
