Pediatrician letter needed as identity proof to get SS Card for newborn

Hi,
Social security office asked me to provide pediatrician letter as an identity proof for my kid. Can you please help me to provide any sample letter/content on this matter? Thanks in advance.

They asked for baby's discharge report but I have only mother's. I am not sure if hospital issues baby's discharge report also :confused: . I have only birth certificate which has needed details.

Hospital should be able to give you 'temporary birth certificate'. You can use that to get SSN for baby.
