My husband plans to get his father, mother and sister on a Visitor visa.

a) Do were require to send three I-134 forms?
b) If yes, then do we require to send multiple copies of the bank statement, employment papers.
c) If my husband applies for his father, then in question no 3 does he have to mention his mother's name as spouse and my sil as the child. I have a confusion regarding question no 3
d) There is question regarding dependents. does my husband have to mention dependents in US and dependents in India, as his parents are dependent on him
financially.

c) Yes for mother. Yes for mother only if SIL is below 18, else no.
d) Only children under 18 and spouse are dependents, not parents. (Even though parents are really dependent, you don't write that here.)
