Quest 39 and 40 on D/S 0156

My sister will be filling out DS 156 for parents. Should she mark yes on 39th question of the forma and write her name on 40th?

And you should copy those questions here while asking them. Nobody can remember the questions. Someone has first look up those questions in the form, and then reply, which is not straightforward. Many people would avoid answering questions if they have to look around to understand them.
