When entering traffic citation (speeding/accident) etc., do we need the exact date of the offense? Or month/year is enough? Who'll remember the exact date of a citation? Not a particularly celebratory event is it? :)

If you remember the exact date or can somehow find it by looking at the prior records, write it. Otherwise, write month/year.

What do I actually have to enter? Violations with court appearance (things like speeding tickets, or running a stop sign), or also accidents that I was at fault in, or all that I was involved in?
The form asks for ever, which is a longish time :-)
I am not sure that I can remember all things I might have been accused of? What do people generally do to reconstruct this, if it should cover 25+ years? (I've lived in the US on and off, and frequent moving certainly helped to lose/destroy records...)

Go to the local DMV and get the Driver's Record and it should have all the information on it for the last 7 yrs.
