N-14 form - bank letter

I have a question about form N-14.
I got this form as a list of documents which I should bring to the next interview.
As first one, there is a marked line "Bank letter showing bank account for years 2003-2008".
Is someone can tell me what kind of information this bank letter should include ?
I called my bank, and the lady ask me this question and I really didn't know what I should say to her. She said that they can send me back all monthly statements since year 2003 and this will be cost me $5 for each month.
I'm not sure if it is what Immigration officer expect to see.

The Bank letter disclosing the info on Acct holder, date it was opened and for how long the acct was active. average monthly balance .......hope this will help. I would advice you better call the USCIS customer service and get more details on what they want?

I called USCIS customer service and they told me that they don't know what suppose to be within this letter.
Gosh what a bullshit. If they don't know how I can know ???
