W7 - ITIN - Last Name change new passport

Hi,

I am on H1b and my wife is on H4, my wife came to USA immediately after marriage, her passport was in her maiden name and visa is also in her maiden name, after coming to usa we got a new passort for her from indian embassy in USA with the changed last name, now
1 ) which passport copy should i send to for ITIN alongwith W7 and
2) what pages of the passport should i send.

When dealing with the IRS you have to make it very easy for them. You have probably taken care of this issue but for other readers, I suggest sending certified copies of both and writing a cover sheet saying "This passport has my maiden name" and another cover sheet that says "This passport has my married name, changed at the U.S. embassy in the states"
