Tax forms F-1 (OPT) to H-1B

Hi,

I have changed status from F-1 with OPT (since Jan 1, 2004) to H-1B (beginning Oct 1, 2004).

Which federal tax forms should I fill 1040NREZ/1040NR ?

I am from state of Arizona, which state tax form should I fill, 140 NR?

I have been filing 8843 when I was in F-1. Should I file it this year also because I was in F-1 with OPT?

Please advise.
