Prepare an amendment to your copy of the DS-160, showing corrections for every item that is wrong. Then write an explanation for how these errors occurred, but do not indicate that you did not review the form, as there is a digital record of your acceptance of all info on the form.

Hold your amendment and explanation in readiness until you have to provide any similar information or start a new visa-based procedure in the future, then include your amendment with that new documentation.

If you inform USCIS now of the errors, you are "muddying the water," and providing a reason for possible negative adjudication of your current visa status. My advice, and not legal advice, is delay informing USCIS of the errors until you need to provide new paperwork to the State Department or USCIS in the future. This is not intended as advice to "withhold" information from USCIS and the State Department, but rather to avoid an unnecessary reopening of your existing visa, which immigration staff may feel compelled to do once your corrections reach someone's desk.

        Saath,

I am not an attorney, but I respond to queries based on considerable experience with immigration procedures.

Your amendment to the incorrect DS-160 can be in the form of an affidavit or a simple statement IN VERY CLEAR ENGLISH, identifying yourself, describing the error, and describing the correct information.
