    1) If your school is located at the same province where you live then you don't need to put it, otherwise yes. This is usually used for Police Clearance Certificate.
2)Yes you need to put tyou current school, it is being attended.

Regarding the form edition, don't panic, it is ok. For more details visit www.state.gov , look for forms.

    Good Luck

I have a question for you man, you sound very panicked, DO YOU HAVE BIRTH CERTIFICATE? IF YES THEN FILL OUT ACCORDING TO THIS B/C, IN OTHER WORDS:

First Name means: your personal name as given in your Birth Certificate.
Middle Name means: your father name as given in your Birth Certificate.
Last Name means: Your Family Name as given in your Birth Certificate.

Look, I filled out Ds-230 back in 2005 for my wife and son. what you need to fill their is the names as they appear on your BIRTH CERTIFICATE. There is another line in DS-230 I guess asks you to fill out if you have names other than the one you put, go ahead and put the one on your school ID there if you wish.

THE IMMIGRATION IN THE US WILL LOOK AT YOUR BIRTH CERTIFICATE AND TAKE THE NAME FROM THERE MAN!!!!!! With this hope things will become clear man....you make it hard on yourself...why is that...again put the names as they appear on your BIRTH CERTIFICATE.
