Hai ,
I want to apply DS-160 forms for my parents . My mothers surname in her passport and all other important documents is her surname after marriage. While filling the DS-160 for my father in the spouses details section it is given in brackets to include maiden name .So should I write her maiden name in the surname column or her surname in passport ( surnameafter her marriage which is in her passport )?
Please guide me .

You should use the name as per passport.
