New address on Indian Passport renewal..

Hello,

I am going to apply for the renewal of my indian passport at San Francisco. I am a US permanent resident . My permanent address in India changed few years ago. Do I have to show proof of change of address in India for passport renewal? Also, is it true that the US address is printed on the new passport? Please help

Also, should the name on every 'name' section of the passport (whether parents', spouse's name), be as on the passport ? i.e. Surname Given Name (s) ?
