US travel medical insurance for Indians ( Australia residents )

Hi Friends

My wife (age 31) and daughter (age 3 ½) is visiting USA for 6 months

Indians but living in Australia so I guess ICICI Lombard is out of choice

Can someone suggest suitable US insurance?
My expectations are
1. Comprehensive
2. Insurance company pay directly to hospital (I am afraid that it they end up in hospital for 2 days, do I need to pay 20K out of pocket and get it back from insurance company!)
3. 100% payable within network and at least 90% outside
4. Refundable in case of early exit

With Patriot Platinum or Atlas America, Do we need to pay upfront in hospital or they just need insurance number and hospitals will bill insurer? How the process works ?
