Blank template for translation of Birth Certificate

My wife and I need to get her Birth Certificate translated from Spanish to English. Do I need to use some sort of template for the translation? I can't find a blank template online, and I don't know how I would format the translated document. Any help would be great...

I would assume it’s appropriate to translate it exactly as it is written on the certificate, including the format. I had a document translated by a translation company and they basically wrote the translation in the same way as the original document (i.e. so the position of headline, numbers and lines matched the original).

You don't need any kind of template for any translation, you just need to hire the certified translator for the translation of your wife's Birth Certificate.
