UK Family Visitor (long term)

Hello,

Based on new changes in application for UK visitor Visa, I following questions if someone can help me?

1) being living in US do I need a Tuberculosis test, since my origination is from India ( here on H1 B visa)
2) I live in Charlotte, NC. Where do I need to send my complete application?
3) Do I need to send a self - addressed envelope? If yes than what kind?
