My mom has interview on Feb 9th at Chennai consulate, We took appt thru tts website long time ago. few q's

1) Does she need to take 3 separate DD's for Rs 4600/2300/441. Con she take them in HDFC bank?

2) Best way to write personal affidavit regarding property(any samples?)
3) Best sample affidavit letter: she is widowed, home maker, has 2 daughters in India, all married and one daughter in US which is me( I am a physician and can support my mom). She has 5 sisters and 1 brother who are all married and are in India. Shouls she has to mention about them? My mom also has her father living independently, can she mention his name as a family tie? If so what supporting documents she has to show?
The sample affidavit in main page immihelp is not suiting her. Appreciate anyone's help.
