I am filling out affidavit of support (Form I-134) for my wife and kid. I want some inputs on question 10...Have you ever filed a petition for visa for anyone....?
My wife and kid are on H-4...Does that mean I should answer 'Yes' to this question ?

Thats does not mean it. It means if you have filed fpr somebody else like your parents.
