Change of name during visa processing

Hello forum,
I was not sure whom to contact in this time of indecisiveness but for the great people out here in this forum.

Background:
My wife is currently on a H-4 visa. Her H-1 has been approved recently and the document is yet to arrive by mail.

Now, the question:
1. Her passport is a little damaged and she plans to apply for a duplicate passport. Will she have a problem changing her passport while her visa is approved.
2. Also, since we got married in a hurry recently (just as many Indians do ), she did not have time to apply for a name change. Is it advisable for her to apply for Change of name alongwith the application for duplicate passport. Or does this create any problems for her in future?

I would really appreciate your help on this matter as early as possible.
