L1B Extension Queries (Travel to India in extension period)

Hi,

I am on L1B visa, Visa Expiry - Jun 2018 but I94 & Petition is expiring in June 2016. My company (well known company) will file my extension case with USCIS in Mid of May 2016.

My Queries are as below:
1. I am not trying to travel to my home (i.e. India) during extension period. But, due to some health reasons, I may need to travel in next 2 month i.e. June or July 2016
What will happen to my extension case filed with USCIS? Will it be rejected?

2. I understand staying in US is important when extension case is filed and result is awaited. but health is something I can not ignore. I understand I cannot travel/return back to US if I travel to India in extension period as my I94 is abandoned. Any chances of getting it approved in case I am staying in India & not US (extension application period)?

3. How my return to India impact my extension case. Do Immigration or USCIS come to know about my exit and they take some action (like rejecting extension case).

Pl. let me know your opinion on this.
