J1 waiver application procedure- help

Hi all
My j1 status for 2 years expires on apr-2017. i want to apply before that. i know that it has been made online these days.
i am trying to get help with lot other websites and previous posts but it has been so confusing as it is online now. can some one guide me with the step by step current procedure and also could you provide me with the links where i can get the required forms.
