Urgent I539 questions - COS from H1 to H4

Hi,

I am planning to change my status from H1 to H4 at the start of next month and filling out the form I539 for same.

Here's the current situation-
Me: Approved I140 from a previous employer, current H1B - I797 valid till 07/2018, changing status from H1B to H4
My spouse: Current H1B-I797 valid till 09/2019

Questions: I have doubts about what to fill in for few questions in the form and would greatly appreciate any help
