Queries about H1B visa

Here is my case:
1. For first time I got H1B vise in 2012. It was valid till 2014.
2. I worked in US from Dec 2012 to May 2014, nearly for 18 months. Then I returned to India.
3. My first visa expired on Oct 2014.
4. In 2016 my same employer again filed my H1B visa in Cap Exempt category.
5. I got visa in Nov 2016. But this time they gave me visa valid till Oct 2017, only for 10 months.


My questions:
1. How come this time my visa is valid till Oct 2017? Though I have applied for 3 years.
2. Can I get visa extension beyond Oct 2017?

1. Visa validity is upto USCIS. There are many a times they approve a petition for 1 year even if employer asks for 3 years.

1. If I get visa extension beyond Oct 2017, will I get new I-797B with extended dates?
2. During extension period, if I visit India, would I require to get my visa stamped again before entering US?
