Maiden name on passport/Married name on Greencard

I am planning to travel to Poland soon however, my passport has my maiden name on it and my green card has my married name.
Will I be able to travel to Poland and back to USA ?? Should I bring my marriage certificate with me just in case??
Also, what last name should I put on the ticket??
