Applying PIO card in Indian Consulate at NY, please help.

Hi,

I have decided to send application form and all other documents for PIO card for my son to Indian Consulate in New york through post. I understand that I have to send notarized copies of his parent's passports. Please clarify the following,

1. Should I need to get notary on all pages? If it is okay to get notary on certain pages of passport please mention.

2. Should I need to get notary on all other documents like xero copy of baby birth certificate, address proof documents, etc...?
