B1 visa Extension

Hello All,

My brother is in B1 visa, He has I 94 till Feb 2007.

We want to apply for B1 Extension.

What is the form we should use ? and what is the supported doc need for B1 extension?

Also what is the services center to send the B1 forms (He lives in kentecky)
