confused abt medical insurance

hi,

my parents r visiting me for 6 months and i am searching for suitable medical insurance for them... i have gone through several of them and pretty much confused which one to buy...does anyone have idea about patriot america???
if so, plz give me ur experience with it....i have a doubt with these medical insurances....

suppose, an emergency situation comes up how does it work with insurance...
i mean we pay the deductable and coinsurance and rest of it will be taken care by insurance company directly or
we pay the whole bill and then send a claim to the insurance company for reimbursement???
