I have a question about visitors to the US. When a visitor from a country in the visa waiver program enters the US (by plane), must he/she have a return ticket within 90 days (i.e., maximum length of stay) of the date of entry?

Most certainly yes. VWP can not be extended. So there is no reason for not having a return ticket within 90 days.
