Surname change for indian passport holder after Marriage

Hi,

I am holder of indian passport issued at New Delhi, India. Post marriage to my fiance, who is currently residing in Redmond, Washington (USA), I wish to change my surname and also add my husband's name in the passport.

Kindly advise what will be the applicable fee for the same. I understand that I would have to send my passport to the nearest Consulate which happens to be San Fransisco in my case. Also if you could confirm the required documents for the mentioned change.
