Previous Exp papers / certificates lost going for Visa Renewal / H1 extn

Hi,

I am right now in US for the last 2 years, am going for visa renewal stamping ( H1 Extn ),i lost my certificates and work exprn letters.An how i managed to get the duplicate copies my certificates and immediate previous employer and with the current empoyer for 4 yrs ( MNC,BEP).Wheni tried reaching my first employer,he seems to be closed his shop.So i could not get any doc from him.

Can i leave blank the DS-157 previous employer 2.
previous employer 1 ( just before the current company ) - ihave doc.
prevouis employer 2 ( first company where i started my career ) - No doc.

As my DS-157 for the first time stamping has both of information.

Please help.

So can i have the same info as the last DS-157 and if the Vo asks for the first companys employment letter,can i say i dont have it as the company is closed or moved some where.
