Do I need to take separate appointment for H4 visa?

I am going for H1B stamping in last week of Sep 2008, wanted to check, if I can add H4B in the same appointment or I need separate appointment.
2. Do I need separate checks (fee) for H4?

You can only add at the time of booking the H1 appointment. If booking later, it has to be a separate appointment. You would have to pay fee for each visa.

If we are making the appointment at the same time, then also I need to have separate draft for spouse?

What is so difficult to understand in this? A visa fee has to be paid per person.
