Urgent !!! Lost EAD in mail, please advice

Hi
My wife's EAD was lost in mail. Her current EAD expires end of Sept.
1.On the I765 form what should she refile it for, so that would make her application faster
a.Replacement (of lost employment authorization document) OR

b.Renewal of permission to accept employment (attach previous employment authorization document).

2. While going through her notice of action that we received, her name is not correct (wrong middle initial).So there is a administrative error made by USCIS.
(As per USCIS website Incorrect Card: No fee is required if you are filing only because the card issued to you was incorrect due to a USCIS administrative error.) Can I ask them for this, so I dont have to pay the fees, but also I dont have the card if they ask for proof

Urgent help would be appreciated.
