Question on Visitor visa extension

Hello,

A few questions on visitors visa extension. My parents have a 10 yr U.S visitor visa and are in their 5th month of stay here.

I. I am planning to apply for a visa extension only for my mother. Will there be a rejection since only one of the two people who travelled together and got their visitors visa together is applying for an extension? Is it better to apply for an extension for both my parents?

II. The visitors visa extend page states that the original I 94 need not be submitted even though the form requires that it be submitted. Is this still valid?

III. Are there any sample cover letters for visa extension?

I. You can either apply only for 1 person or both. Does not matter.

II. Send only copy of I-94.
