Hi all,
I recently became US citizen and based on my marriage certificate I changed my last name to my husband's.
when I went for citizenship interview the lady said there is no no need to goto court for name change as name change is based on marriage certificate.
so she changed my name herself.
I tried to appy for PIO card in DC and today I got a call from the embassy requesting for a court order to prove that my name has changed from A to B.
I told her I have provided the naturalization certificate which shows my new name and passport/marriage certificate for old name but she said she needs some kind of proof to show that my name was indeed changed from A to B.
( i don't think nowhere they say my previous name and new name together)
I don't know whether I can goto court as already my name is changed.
can I get a signed affidavit from notary?? if so, what affidavit form can I use and can it be any notary?
or is there any alternate solution for my case.
Please advise me if you have any experience/suggestion about this.
TIA!!
