    Default regarding Brith Certificate

My twin babies born in US 7 months ago and currently they are holding US citizenship and passport. In order to get their Indian Citizenship, I found from google that, If there name can be registered in US consulte within 1 yr from birth then they will get the indian citizenship.

For that I have download the Miscellaneous Services form, from the consulate site. But while filling it up I found few quires. Please help me to clear my doubts.

    1. Who will be the applicant? Would it be the father or the children?
    2. In section II, can I mention both of my Children names? If so, how much would be the service fee?
    3. Other than the form, what are the other documents, I need to provide.
    4. Should I be sending separate Cashier Check for the return mail charge or it can be included in the same check for service fee.

Your help will be much appreciated.
