If someone applies for a visitor visa extension along with filling out the entire form properly they also need to include what?

The whole photocopy of his/her passport?

Photocopy of return ticket? What if the date for the return ticket already passed because they stayed beyond it,not overstaying the I-94 stamped date. They have to purchase another ticket before applying and have that ticket as the future date they are applying for?

And what proff of income is acceptable? What if I, being a US citizen and a close friend am paying for everything?
