Regarding affidavit for name correction

Hi,

My mom has an interview at chennai Consulate. In my mom's passport her last name is mentioned diffrently from what they mentoned in my passport (that is mother's name in my passport.)
I am planning to get an affidavit for that. But I do not have a format for that. Anybody please suggest me a format for the same.

You don't need the format.

Just go to local civil court and ask any lawyer roaming around there. They will get it done for you.

Once you do that, please post it over here for benefit of others.
