    Default PIO card NY- Place of birth fields confusion

    I am filling PIO card application (NY consulate) for my daughter.
    For Place of birth there are 3 fields, Town, City and Country.
    Her birth certi has two columns 1 Town/City = Winchester
    and 2 Country = Middlesex

    What to fill in the form?
    Town = Winchester
    City = ?? (Boston??)
    Country = Middlesex OR USA

    Please help.

    Middlesex is the COUNTY where Winchester is located, not the country, and so does not need to appear on the PIO application.

    Since Winchester is not officially or legally part of a larger city (though it may generally be considered part of metro Boston), you don't need to fill in both Town and City areas -- just use one or the other.

    Remember that these forms are trying to cover any and all address possibilities and may not match up with your location exactly.
