University of Alabama student health insurance

Dear Sir/Madam, I am an international graduate student in The University of Alabama. I am looking for a health insurance for the Fall semester. My university has certain requirements that the insurance should meet. Please find below those requirements, The policy should cover me for medical expenses equal to or greater than $250,000 for each illness/accident. Standard co-insurance of 30% A deductible no greater than $250 per policy year for in network providers or $500 per policy year for out of network providers. Coverage for repatriation of remains is equal to or greater than $10,000. Medical evacuation coverage is equal to or greater than $10,000. If there is a PPO requirement associated with Plan benefits. Is there availability of hospitals and physicians in the greater Tuscaloosa area, AL? The policy meets J visa requirements as set forth by the Department of State.
