Question Urgent: How to fill I-129S for L1 Blanket

I have my L1B Visa interview in 2 days. Just today my HR department told me that I also have to fill Form I-129S. I have found the form at http://www.uscis.gov/files/form/i-129s.pdf.

I have few queries regarding this:
    1. Once I have filled this form, do I just need to take 3 prints of this form for my Visa interview; or do I need to do something else with it too (like mail or upload it somewhere)?
    2. The Form asks for Intended dates of employment. I have filled the From Date as per my Offer Letter. But can I leave the To Date column blank? If I fill it for 1-2 year later date, will I get Visa only for that period or will I get visa for full 3 years term?

    >>>> Just carry the print outs. That should be fine.

    >>> Fill the end date as the petition expiry date.
