OCI application - missing indian passport

I would like to apply for OCI card, but lost / missing Indian passport. I'm US Citizen. I believe I can also use my parents Indian passport, but missing my birth certificate also. How do I move forward?

You have to get certificate that you have lost / missing Indian passport and you have to show every time, every place they will ask you to show cancel indian passport. BTW it's same form and fees.
I may be wrong but you don't need birth cert. for OCI...
