PIO Card- parents documents not available

Hi
I want to apply PIO card for my kid 4 months old. but i have only my visa status,passport documents. i don't have other parent documents. how can i apply for pio card or any other option to travel to india.please help
on the declaration form do they need both parents signatures for minor child?other parent not available. please help
