Default Visitor visa status, how is it known

If visitor Visa is rejected, will there be a stamp in the passport with titile 'rejected'

    or

Do the consulate just give a letter with rejection ?

The reason I am asking is because I want to apply for my Moms visa. Last year she applied for visa but the consulate asked for more documents in the interview. We never bothered to go back and my Mom stayed in India.

This year, I am applying again. I dont know whether I need to check for the question 'Have you ever been refused a US Visa ?'

    Thank you for your suggestion.

If visa is rejected, there is no such stamp. They just put small stamp towards the back of passport saying 'Application received'.

If they just asked you go come back with more documents, I don't think that is rejection.
