L2 Visa questions

hi friends,

I am going to get married soon,My fiancé have L1b visa so that i can apply for L2b, my questions are

1. is L2 visa apply through her company or individually?

2.what are the forms should submit(apart from DS-160)?

3. Can we block interview date before marriage?

4. Is it mandatory to submit my original certificates and work experiences letters at the time of interview?

5. Could you please give a check list of the documents should carry??
