How does surrender certificate look like?

For those of you who have received it, how does it look like? Does consulate simply sign/stamp your application form and return it back to you?

That will be one shady looking certificate with all the typos and ever changing form. May be I was expecting too much from Indian Govt.
They can't design a simple form, with actually looks legit.
